This study investigated autonomic dysfunction in ME/CFS patients by measuring skin temperature rhythms, hemodynamic responses to standing, and endothelial biomarkers. ME/CFS patients had significantly higher levels of the vasoconstrictor endothelin-1 (ET-1) and vascular adhesion molecule VCAM-1 compared to healthy controls, and ET-1 levels correlated with symptom severity. A discriminant model combining blood pressure, endothelial biomarkers, and circadian temperature variables correctly classified 91.5% of participants, suggesting that endothelial dysfunction plays a key role in the autonomic symptoms of ME/CFS.
