In a prospective study of 280 celiac disease patients who reported strict gluten-free diet (GFD) adherence, 11.4% tested positive for gluten immunogenic peptides in urine (uGIP), indicating recent involuntary gluten ingestion. Positive uGIP was significantly associated with duodenal mucosal atrophy on biopsy (66.7% vs. 32.7%, p=0.01), but did not correlate with symptom scores, dietary adherence questionnaires, or anti-tissue transglutaminase antibody levels. The findings suggest that a single uGIP test can identify otherwise undetected gluten exposure and may serve as a useful point-of-care tool for monitoring GFD adherence in celiac disease.
