This bench-to-bedside study tested whether volatile organic compounds (VOCs) linked to oxidative stress in cell experiments could be validated in ventilated ICU patients exposed to high oxygen levels. Despite clear cellular oxidative stress, none of the known or newly identified VOCs (including octane and 2-ethyl-1-hexanol) correlated with patient oxygen exposure or discriminated high-risk patients. The findings highlight major challenges in translating exhaled breath biomarkers into clinical practice and caution against assuming oxidative stress as the origin of detected VOCs.
