Ultrahigh resolution OCT reveals a hyporeflective split within the RPE-BL-BrM complex that decreases with normal aging but reappears and is significantly thicker in early AMD eyes, corresponding histologically to basal laminar deposit (BLamD). This split can serve as an in vivo imaging marker to differentiate normal aging from early AMD pathology. The findings enable quantitative assessment of early AMD features previously only accessible ex vivo, with potential to facilitate drug development and clinical trials.
