This cross-sectional study analyzed 202 GRADE-based recommendations from 8 Irish National Clinical Guidelines and found that 63.6% of strong recommendations were based on low or very low certainty evidence (discordant). Although the majority of these discordant recommendations could be mapped to one of five GRADE paradigmatic situations justifying such discordance, none of the guidelines explicitly referenced these situations. The findings highlight a need for greater transparency and training in guideline development to ensure trustworthy recommendations.
