This case report describes the first long-term nutritional management of an obese German Spitz with paroxysmal dyskinesia (PD), calcium oxalate urolithiasis, and suspected pancreatitis. A gluten-free, hypoallergenic, low-fat, low-relative-supersaturation diet led to a marked reduction in neurological episodes, allowing gradual discontinuation of anti-seizure drugs, with relapses linked to dietary indiscretion or higher-fat diet changes. The findings support a possible dietary association with PD, though a causal relationship with gluten could not be confirmed.
