This pilot randomized controlled trial found that a single oral dose of melatonin (0.5 mg/kg) given to newborns before surgery significantly reduced oxidative stress markers (NPBI, F2-isoprostanes, and AOPP) in the postoperative period compared to untreated controls. Endogenous melatonin levels rose physiologically after surgery but were insufficient to neutralize surgical oxidative stress. Exogenous melatonin supplementation may protect neonates from oxidative damage associated with surgical procedures.
