This in vitro study examined triheptanoin effects on fibroblasts from five patients with Neutral Lipid Storage Disease Type M (NLSD-M), an ultra-rare ATGL deficiency disorder. Triheptanoin significantly improved mitochondrial respiration parameters (basal, maximal respiration, spare respiratory capacity, and ATP production) and stimulated glycolysis in patient fibroblasts, while normal controls showed no changes. However, triheptanoin did not reduce intracellular triglyceride accumulation, indicating it provides anaplerotic metabolic support without addressing the underlying lipolytic defect, supporting its potential as adjunctive metabolic therapy for NLSD-M.
