This study of 120 exclusively breastfed infants found that higher 5-MTHF concentrations in breastmilk were associated with lower maternal plasma folate (suggesting lactation depletes maternal folate stores) and with higher folate status in infants at 4 months of age. However, neither breastmilk nor maternal plasma folate concentrations were associated with infant growth (weight, length, or head circumference z-scores) during the first 4 months of life. The findings suggest that adaptive mechanisms may buffer the effects of low milk folate on infant development, and that folate supplementation during lactation may help prevent maternal depletion.
