This randomized controlled trial in 40 adolescent boys with NAFLD found that an 8-week low free-sugar diet (LFSD) significantly reduced hepatic fat (from 25% to 17%) compared to usual diet (21% to 20%), independent of weight loss. Plasma metabolomics revealed differential changes in 419 HILIC and 205 C18 metabolite features enriched in amino acid and lipid pathways, including increased serine, glycine, and 3-hydroxybutyric acid, and decreased linolenic acid. Microbiome analysis in a subsample showed increased phylum-level richness and changes in Firmicutes genera, suggesting that sugar restriction may improve NAFLD through mechanisms involving one-carbon metabolism, ketogenesis, and gut microbiota composition.
