This retrospective study of 348 nasopharyngeal carcinoma (NPC) patients found that a low serum transferrin (TRF) level before intensity-modulated radiotherapy (IMRT) was significantly associated with lower radio-sensitivity and worse overall survival. Patients with TRF ≤ 2.16 g/L had a radio-sensitivity rate of 64.6% versus 74.6% in the high-TRF group (p = 0.043), and a 5-year overall survival of 73.1% versus 84.4% (p = 0.02). Serum TRF may serve as a simple, clinically accessible biomarker to predict treatment response and prognosis in NPC patients undergoing IMRT.
