This prospective randomized study compared tunica albuginea plication (TAP) and corpus cavernosum rotation (CR) for penile curvature correction in 22 patients. Both techniques effectively straightened the penis and improved erectile function, but TAP caused greater penile shortening (mean 2.4 cm) while CR caused greater reduction in girth (mean 1.2 cm). TAP is not recommended for patients with preoperative curvature >60 degrees due to a 2.7-fold increased risk of subjective penile shortening.
