Paracetamol's analgesic effect is primarily mediated by its central metabolite AM404, which activates TRPV1 channels, Cav3.2 calcium channels, and CB1 receptors in the brain—a mechanism independent of cyclooxygenase (COX) inhibition and distinct from NSAIDs. This centrally acting, COX-independent mechanism explains paracetamol's favorable safety profile, including lack of gastrointestinal, renal, and cardiovascular adverse effects, making it the preferred first-line analgesic for mild-to-moderate acute pain in healthy adults and special populations such as the elderly, patients with renal impairment, cardiovascular disease, or gastrointestinal risk. Future paracetamol analogs targeting the central TRPV1 pathway without producing the hepatotoxic metabolite NAPQI are in development and show promise for improved benefit-to-risk ratios.
