This cross-sectional study of 406 people who use or inject drugs (PWUD) in Kachin State, Myanmar, found that 32.8% had symptoms of anxiety or depression and 61.8% had experienced physical or emotional violence in the past year. Only forced displacement due to war or armed conflict—not economic migration or migration for any reason—was significantly associated with both poor mental health (aOR 2.33) and violence (aOR 2.18). The findings highlight the urgent need to integrate mental health services into existing harm reduction programs and address broader social determinants like food insecurity, unstable housing, and stigma.
