This cross-sectional survey of 68 North Carolina cardiac rehabilitation programs (89% response rate) found that while 94% assess mental health at enrollment and 70% offer psychosocial services, the majority (73%) do not directly bill for these services. Program directors identified lack of internal resources (92%) and patient beliefs (45%) as the top barriers to expanding psychosocial care. The findings highlight a critical gap between routine mental health screening and the availability of resources to deliver comprehensive psychosocial interventions in cardiac rehabilitation.
