This feasibility study of 41 patients undergoing colorectal cancer resection found that perioperative high-dose vitamin D3 supplementation (3200 IU/day) was safe, well-tolerated, and significantly increased serum 25OHD levels while reducing post-operative vitamin D insufficiency compared to 81 controls. Supplementation also attenuated the post-operative drop in 25OHD and was associated with lower early post-operative C-reactive protein levels, suggesting an anti-inflammatory effect. These findings support early initiation of vitamin D supplementation at the time of colorectal cancer diagnosis, though randomized trials with survival endpoints are needed.
