In infants with antenatally diagnosed tuberous sclerosis complex (TSC) who met criteria for prolonged video-EEG (vEEG), seizure frequency was high (average 1 focal seizure/hour) but only 32.6% of seizures had clinical manifestations. Parents identified only 28.6% of clinical seizures despite ongoing education, and no epileptic spasms were detected by parents. These findings indicate that treatment decisions should not rely solely on parental seizure reporting and that prolonged vEEG is important for accurate seizure assessment in this population.
