A 5-month-old exclusively breastfed infant presented with severe zinc deficiency (undetectable serum zinc <3 μmol/L) and classic acrodermatitis. A novel pathogenic variant in the maternal SLC30A2 gene (c.927G>C) was identified, which impairs zinc secretion into breastmilk. The case suggests that maternal proton pump inhibitor (omeprazole) use during the third trimester may have further reduced zinc availability, precipitating symptomatic deficiency in the infant.
