This mixed-methods study explored parental attitudes toward infant feeding and tested a text-message intervention to promote healthy feeding practices and prevent early obesity in a low-income, diverse urban population. The pilot randomized controlled trial found no significant differences in weight-for-length percentiles, breastfeeding persistence, or introduction of solids between the intervention and control groups, though the study was underpowered due to low survey response rates and loss of 12-month data from the COVID-19 pandemic. Text messaging remains a promising avenue for extending provider communication, but future interventions should incorporate social support elements and flexible contact methods to address barriers like phone service interruptions.
