This cohort study of 1691 mother-offspring pairs found that children and young adults born preterm (<37 weeks) had significantly higher health care utilization related to COVID-19 concerns compared with those born at term (51.1% vs 33.6%). Individuals born extremely preterm (<28 weeks) had twice the odds of seeking care (aOR 2.15; 95% CI 1.40-3.29), driven largely by respiratory symptoms. These findings suggest that preterm birth history is an independent risk factor for pandemic-related health care use and may help refine care models for vulnerable pediatric populations during public health emergencies.
