Chronic kidney disease (CKD) affects nearly 40% of people aged 65 or older, yet few evidence-based guidelines specifically address its management in this population. This review advocates for individualized care using the Kidney Failure Risk Equation (KFRE) to identify high-risk patients, while emphasizing less aggressive targets for frail older adults to avoid polypharmacy and adverse events. Clinicians should engage in shared decision-making about renal replacement therapy, as dialysis offers modest survival benefits in older frail patients and conservative management is a reasonable alternative.
