This case report describes a 16-year-old female on ventricular assist device (VAD) support who presented with hematemesis and was successfully evaluated using video capsule endoscopy (VCE) after endoscopy was deemed high risk due to pulmonary hypertension. VCE identified the gastric antrum as the bleeding source without device-related complications, allowing conservative management with intravenous pantoprazole. The findings suggest VCE is a safe and effective diagnostic tool for GI bleeding in pediatric VAD patients, though further studies are needed to establish its role in this population.
