This nested cohort study within the ELFIN randomized controlled trial found that supplemental bovine lactoferrin had minimal impact on the gut microbiome and metabolome of very preterm infants, explaining less than 3% of variance in microbial composition. In contrast, NICU site (explaining up to 40% of variance) and postnatal age were the dominant drivers of gut microbial and metabolite profiles. These mechanistic findings align with the parent trial's null clinical result, suggesting that single-nutrient supplementation may be insufficient to alter the preterm gut ecosystem or reduce sepsis.
