This cross-sectional study of 1,014 participants in India found that low-level arsenic exposure (mean urinary arsenic 9.65 µg/L, below WHO drinking water thresholds) was associated with a syndrome of executive function deficits including impaired attention, working memory, set shifting, cognitive flexibility, and increased risk-taking. These cognitive impairments were mediated by structural brain changes (reduced gray matter volume in inferior frontal cortex, insula, anterior cingulate, and inferior temporal cortex) and disrupted functional connectivity, with effects exacerbated by low socioeconomic status and low BMI. The findings suggest that current safe exposure limits may need reexamination and that improving nutrition and socioeconomic conditions could mitigate neurocognitive harms from arsenic.
