This retrospective study found that approximately 1 in 4 previously healthy infants under 3 months of age had CT evidence of hepatic steatosis, suggesting nonalcoholic fatty liver disease may begin in early infancy. Despite this finding, none of the original clinical radiology reports mentioned steatosis, indicating significant underdiagnosis. The clinical significance is that current screening guidelines starting at age 9–11 years may miss early disease onset, and further research is needed to understand whether infantile steatosis progresses to severe pediatric NAFLD.
