This case report describes a 5-year-old child with autism who presented with severe gastrointestinal bleeding and jaundice, ultimately diagnosed with metachromatic leukodystrophy (MLD) after finding gallbladder polyps causing hemobilia. The bleeding required multiple transfusions and a biliary drain, and cholecystectomy revealed extensive polyps characteristic of MLD. Clinicians should suspect MLD in children with developmental regression and gastrointestinal bleeding, and monitor known MLD patients for gallbladder complications.
