This case report describes a 17-year-old boy with Crohn's disease who experienced a seizure during an infliximab infusion, associated with undetectable drug levels and moderate anti-TNF antibodies (15.2 U/mL). The seizure resolved without recurrence after switching to adalimumab, suggesting immunogenicity may play a role in neurologic adverse events. Clinicians should consider monitoring drug and antibody levels in pediatric patients on infliximab who develop infusion reactions.
