This retrospective study of 20 Brazilian children with very-early-onset inflammatory bowel disease (VEO-IBD) found that ulcerative colitis (65%) was more common than Crohn disease (35%). Children with Crohn disease were significantly younger at symptom onset and had more severe nutritional impairment, anemia, and higher inflammatory markers at diagnosis. The study highlights the aggressive clinical course of VEO-IBD in this population, with high rates of perianal disease in Crohn disease and two cases of lymphoma in children on combination immunosuppressive therapy.
