This case series of 7 toddlers shows that excessive cow milk intake (≥24 oz/day) can cause protein-losing enteropathy (PLE) with severe iron deficiency anemia (IDA), presenting as edema. All patients improved with cow milk restriction and oral iron supplementation, with edema resolving and anemia improving at follow-up. Clinicians should consider IDA and PLE in toddlers with anasarca and a history of high cow milk consumption.
