In a large trial in Malawi, a single intravenous dose of ferric carboxymaltose did not reduce anaemia at 36 weeks' gestation or improve birthweight compared with standard oral iron in pregnant women with moderate-to-severe anaemia. Although it safely corrected iron deficiency and iron deficiency anaemia, overall anaemia remained high, likely due to non-iron causes such as inflammation. These findings suggest that in malaria-endemic settings, intravenous iron alone may not be sufficient to overcome the complex causes of anaemia in pregnancy.
