**Background:** Foodborne illness is caused by consuming contaminated food or water, with over 31 documented pathogens. In the U.S., Norovirus accounts for 58% of cases, nontyphoidal Salmonella 11%, Clostridium perfringens 10%, and Campylobacter spp. 9%. Other causes include agrochemicals, climatic changes, and improper food handling. Despite preventive measures, foodborne illness remains a significant public health threat, with changing food production practices, intensive farming, and antibiotic resistance contributing to the emergence of new pathogens like Listeria monocytogenes and multidrug-resistant Salmonella. The federal government estimates 48 million cases annually (1 in 6 Americans), leading to 128,000 hospitalizations and 3,000 deaths. Outbreaks are more common in the Southern U.S. due to low socioeconomic status. From 2017 to 2020, the CDC reported 65 outbreaks caused by Salmonella, STEC, and L. monocytogenes, with 52 linked to contaminated foods, affecting 7,659 people (average 1,900 per year), 2,044 hospitalizations, and 41 deaths. As of September 2022, cases of Salmonella (144), L. monocytogenes (20), and E. coli (97) indicated an 8% rise in overall foodborne cases in just one month compared to average annual incidence.
**Methods:** This is a short communication (narrative review) summarizing trends in foodborne illnesses in the U.S. based on published literature and CDC data. No original data collection or statistical analysis was performed.
**Key Results:** The paper highlights a surge in Campylobacter and Shigella infections since 2015. From 2017 to 2020, 65 outbreaks from Salmonella, STEC, and L. monocytogenes resulted in 7,659 cases, 2,044 hospitalizations, and 41 deaths. As of September 2022, a total of 144 Salmonella, 20 L. monocytogenes, and 97 E. coli cases were reported, reflecting an 8% increase in one month. The paper also notes that frequent dining at fast-food restaurants, processed foods, intensive farming, biofilms, imported contaminated food, and unsafe agricultural practices (e.g., organic waste as fertilizer) contribute to the rising incidence. Antibiotic resistance has led to multidrug-resistant Salmonella strains.
**Clinical Implications:** Healthcare providers should educate vulnerable populations (immunocompromised, pregnant women, young children, elderly) about food safety. Patients with blood in stool, hematemesis, prolonged diarrhea (≥3 days), severe abdominal cramping, or high fever should seek immediate medical attention. Antibiotics should only be used on practitioner advice. Preventive measures include hand washing, cleaning utensils, checking expiry dates, avoiding raw/undercooked poultry/meat/eggs, drinking boiled water/milk, and discouraging reheated food. Government provision of water filters in rural areas is recommended.
