This study used mass spectrometry imaging to analyze sulfatide lipids in intrahepatic cholangiocarcinoma (iCCA) tumor tissue and found that while total sulfatide abundance per pixel was similar to normal bile ducts, a high ratio of unsaturated to saturated sulfatide species was associated with significantly shorter disease-free survival (10 vs. 20 months). Sulfatides were also detected in colorectal liver metastasis (markedly elevated) and hepatocellular carcinoma (distinct odd-chain signature), suggesting tumor-type-specific lipid profiles. These findings identify the sulfatide saturation ratio as a potential prognostic marker in iCCA and warrant validation in larger cohorts.
