This narrative review summarizes current pharmacological treatments for diabetic peripheral neuropathy (DPN), including SNRIs, gabapentinoids, and TCAs, and highlights emerging therapies such as GLP-1 agonists and mirogabalin. Up to 25% of DPN patients experience pain, and current monotherapy fails to provide adequate relief for about two-thirds of patients, underscoring the need for new treatments. The review concludes that while GLP-1 agonists show promise in preclinical and small clinical studies, large-scale trials are still needed to confirm their efficacy in DPN.
