Oral zeaxanthin 20 mg daily in unilateral neovascular AMD patients reduced 5-year fellow-eye conversion from 48% to 22% (p<0.0001) compared to historical CATT controls. This yielded a dominant cost-utility ratio (−$576/QALY direct ophthalmic, −$125,071/QALY societal) and a 1531% return on investment over 11 years. The findings support zeaxanthin as a cost-effective preventive therapy for second-eye neovascular AMD.
