γ-Aminobutyric acid (GABA) is the main inhibitory neurotransmitter in the brain, and enhancing GABAergic inhibition is a key mechanism for many antiseizure medications. Two recently approved drugs, cenobamate and ganaxolone, act via GABA modulation: cenobamate shows robust efficacy for refractory focal seizures (seizure freedom rates up to 21% in pivotal trials), while ganaxolone is the first approved therapy for seizures associated with CDKL5 deficiency disorder. These agents expand treatment options for drug-resistant epilepsy, though their use requires careful management of drug interactions and adverse effects.
