Rural older adults in China perceive health as the ability to perform daily chores, prioritize symptom relief over disease cure, and avoid healthcare to preserve their established life routines. Despite convenient access to healthcare, they prefer informal self-treatment and selectively comply with medical advice. Healthcare reform must address subjective accessibility and culturally adapt health education to improve utilization.
