This case report describes a 10-year-old boy with a novel hemizygous loss-of-function TLR7 variant (c.1386_1389dup, p.His464Ilefs*7) who developed severe neurological deterioration after a mild COVID-19 infection. The variant likely impaired type I and II interferon responses, contributing to progressive neurological decline despite immunotherapy. This finding suggests that TLR7 deficiency may be a risk factor for post-COVID-19 neurological complications in young, previously healthy patients.
