The Cellular and Organismal Effects of Nitroxides and Nitroxide-Containing Nanoparticles

**Background:** Nitroxides are stable free radicals with antioxidant properties that catalytically decompose superoxide (SOD-mimetic activity), scavenge peroxyl and other radicals, and stimulate catalase-like activity of hemoproteins. Their short in vivo half-life has led to development of nitroxide-containing nanoparticles (e.g., redox nanoparticles, RNPs) with prolonged circulation. This review covers chemical reactivity, cellular effects, animal studies, clinical trials, and nanoparticle formulations.

**Methods:** The authors conducted a narrative review of published literature on nitroxides (TEMPO, TEMPOL, TEMPAMINE, and derivatives) and nitroxide-containing nanoparticles. They synthesized data from in vitro cellular models, in vivo animal experiments, and human clinical trials, focusing on antioxidant mechanisms, protective effects, and adverse effects.

**Key Results:** - **Reactivity:** TEMPO reacts with alkyl radicals at diffusion-limited rates (~1–3 × 10^10 M^−1 s^−1). Piperidine nitroxides (TEMPO, TEMPOL) catalytically dismutate superoxide with rate constants of 1.2 × 10^5 M^−1 s^−1 (TEMPO) and 6.5 × 10^4 M^−1 s^−1 (TEMPOL). Nitroxides also scavenge NO2• (k = 3–9 × 10^8 M^−1 s^−1) and CO3•− (k = 2–6 × 10^8 M^−1 s^−1). - **Cellular protection:** TEMPOL (0.03–3 mM) protected rat pleural macrophages from H2O2 toxicity. TEMPOL (5–100 mM) protected Chinese hamster cells from X-irradiation with protection factors of 1.25–2.5. TEMPO (100–150 μM) increased survival of SH-SY5Y cells treated with 6-OHDA (Parkinson's model). Mito-TEMPO (25 nM) decreased ROS and preserved mtDNA in HEK-2 cells under hypoxia/reoxygenation. - **Animal studies:** TEMPOL in chow (10 mg/g food) increased mean lifespan of C3H mice (123 vs. 92.6 weeks) and decreased tumor incidence (10% vs. 40%). TEMPOL (275 mg/kg i.p.) increased LD50 from 7.84 Gy to 9.97 Gy in irradiated mice. TEMPOL (30 mg/kg) attenuated renal ischemia-reperfusion injury. RNP^N^ (200 mg TEMPO/kg) increased survival of irradiated tumor-bearing mice and prevented kidney/liver damage. - **Clinical trials:** Topical TEMPOL (70 mg/mL) before whole-brain radiation resulted in full scalp hair retention in 60% of patients. OT-551 eye drops (0.45% three times daily for 2 years) preserved visual acuity in dry AMD patients. TEMPOL (10 μM via microdialysis) improved NO-dependent cutaneous vasodilation in smokers and chronic kidney disease patients. - **Nanoparticles:** RNP^O^ had a half-life of 600 min (2400× longer than TEMPOL). RNP^N^ (300 mg/kg i.v.) reduced tumor volume growth in colon-26 adenocarcinoma mice. RNP^N^ in drinking water (5 mg/mL for 6 months) improved memory and reduced Aβ levels in Tg2576 Alzheimer's mice. - **Adverse effects:** IC50 values in HaCaT cells: TEMPO 2.66 mM, TEMPOL 11.4 mM, TEMPAMINE 9.5 mM. TEMPOL (0.2–2 mM) decreased erythrocyte GSH and increased methemoglobin. TEMPOL (1 mM) induced 60–67% apoptosis in rat glioma C6 cells. RNP^N^ and RNP^O^ did not induce hypotension in contrast to free nitroxides.

**Clinical Implications:** Nitroxides, particularly TEMPOL and its prodrug OT-551, show promise as radioprotectors (preventing alopecia and dermatitis) and for treating dry AMD. Their ability to scavenge superoxide and preserve nitric oxide bioavailability suggests utility in cardiovascular and microvascular diseases. Nitroxide-containing nanoparticles (RNPs) overcome the short half-life of free nitroxides and enable targeted delivery to inflamed or tumor tissues, with potential applications in cancer therapy, neurodegenerative diseases, and ischemia-reperfusion injury. The low toxicity and favorable safety profile support further clinical development.
