**Background:** Myopia is a multifactorial vision disorder with increasing global prevalence, projected to affect over 5 billion people by 2050. Prior studies on the relationship between vitamin D levels and myopia in children have been inconsistent and limited by small sample sizes. This study aimed to clarify the association using a large, nationally representative sample.
**Methods:** Data were extracted from the National Health and Nutrition Examination Survey (NHANES) database for participants under 19 years old from 2001 to 2008. Myopia was defined as spherical equivalent ≤ -1.0 D measured by autorefractor. Serum 25-hydroxyvitamin D [25(OH)D] was quantified using high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). Covariates included age, sex, race, education, body mass index (BMI), poverty income ratio (PIR), and leisure-time exercise (minutes/week). Stepwise logistic regression was used in three models: Model 1 (unadjusted), Model 2 (adjusted for sex, age, education, ethnicity), and Model 3 (further adjusted for leisure-time exercise).
