A 63-year-old female presenting with acute abdominal pain was surgically explored for suspected perforated appendicitis, but intraoperative findings revealed chylous ascites secondary to acute pancreatitis with a normal appendix. Drainage and conservative postoperative management led to uneventful recovery.
