This narrative review surveys vision-threatening ocular adverse effects associated with commonly prescribed systemic medications, including corticosteroids, amiodarone, ethambutol, topiramate, tetracyclines, retinoids, niacin, fingolimod, vigabatrin, and anticancer agents. The central finding is that while these complications are generally uncommon, certain medications carry well-characterized risks for irreversible vision loss that warrant clinician awareness and, in some cases, ophthalmologic screening.
