BACKGROUND
COVID-19 is a global pandemic with varying impacts based on social determinants of health (SDH). The Xingu Health Region in the Brazilian Amazon faces unique challenges due to socioeconomic factors and limited healthcare resources. Methods: This observational ecological study used secondary data from March 2020 to March 2021, including 16,138 COVID-19 cases confirmed by RT-PCR or rapid test. Data sources included SESPA, IBGE, e-Gestor AB, and INPE. Statistical analyses involved descriptive statistics, correlation coefficients, binary logistic regression, and Kaplan-Meier survival analysis. Key Results: The incidence rate was 45.59 per 1000 inhabitants, and mortality rate was 1.01 per 1000. Higher incidence was observed in Vitória do Xingu (90.84) and Altamira (56.83). SDH indicators like GDP, health insurance coverage, and public health expenditure were positively correlated with incidence and mortality. Predictors for medical ward admission included age, residence in Brasil Novo, and symptoms like fever, cough, emesis, and dyspnea, along with comorbidities such as diabetes, heart disease, and obesity. ICU admission was predicted by age, imported cases, dyspnea, emesis, chills, and comorbidities like diabetes and heart disease. Mortality was associated with age, residence in Anapu, dyspnea, fever, and heart disease. Survival analysis showed that individuals aged 60 years or older had the lowest survival rate. Limitations: The ecological design limits causal inferences, and there may be errors in data reporting, underreporting, and lack of race/ethnicity data. SDH indicators were sourced from different periods, affecting reliability. Implications: The findings highlight the importance of SDH, symptoms, and comorbidities in COVID-19 outcomes, which can inform public health policies and resource allocation in the Xingu Health Region. However, further studies with individual-level data are needed to confirm causal relationships.
