This study aimed to demonstrate disparities in health information system applications among Indonesian community health centers (CHCs) by province and region. Methods: A cross-sectional analysis was conducted using data from the 2019 Health Facility Research (RIFASKES), involving 9831 CHCs. The mean number of applications was 10.37, with a median of 10.80. Papua and West Papua had less than 60% for all data-storage programs. Some programs like ASPAK, PISPK, and P.care were owned by approximately 90% of CHCs. Limitations: The study did not evaluate the implementation process or data quality, and not all information systems, such as medical records, were assessed. Implications: The findings highlight the need to strengthen health information systems in Indonesia, particularly in eastern regions, by improving resources, human resources, and management support. Recommendations include government evaluation of CHC information systems and addressing interoperability and data quality issues.
