This narrative review summarizes the molecular mechanisms of pain signaling, including nociceptors, excitatory and inhibitory receptors, and glial involvement, with a focus on inflammatory and diabetic neuropathic pain. It concludes that inhibitory receptors like glycine receptors are promising therapeutic targets for novel analgesics.
