This narrative review examines how bacterial metabolites, particularly ammonia and bile acids (BAs), contribute to hepatic encephalopathy (HE) by summarizing evidence from human clinical observations and preclinical animal models. The central conclusion is that dysregulated BAs and their receptors (FXR, TGR5, S1PR2) are important mechanistic players in HE pathology alongside ammonia.
